Injection Boundary for 3D Pulsed Jets

This is related to ticket #289.

It looks like the point at which things really blow up was at a restart (restarted from frame 33, explosion at frame 34), so maybe there is something about my code that makes restarts a bad idea. Regardless, the simulation was at a hault before I tried doing the restart.
